Click on the pdf form you need to submit, save it to your device (Ctrl S or the download button on the upper right of the form), or click the Download button next to the pdf which will load the file to your Download folder, fill it out completely, save it and email it to: claims@oriska.com
Note, forms not filled in completely may cause delays in claim payments.
